Old Cancelled Indian passport Or Current OCI Card (in case of US nationals of Indian origin)

Original Notarized Power of Attorney (POA made by applicant) + 1 Photocopy of notarized POA for Consulate record. Witness signature are mandatory. POA should have USA address in first paragraph. Signature on POA should match with signature on current valid passport. In case signature differs, the documents would be sent back to be signed again, as in the passport.
