For the Beneficiary (Relative Seeking Green Card):

Passport-style photos (2 recent photos)

Copy of passport biographic page and visa stamp (if in U.S.)

Proof of lawful entry (Form I-94) (if applicable)

Affidavit of Support documents (for Adjustment of Status phase)
