  • Current Passport

  • Current USA Visa/Green Card/ Work Permit

  • Proof of address (Driver License or Utility Bill only)

  • 2 Passport size photos

Additional Information (to be sent along with document copies):

  • Address and phone number in India

  • Indian address police station name and city

  • Emergency Contact person and phone number

Step 1

Sign the completed forms (sent by us), and mail them back for further processing

Step 3

Receive your final document in mail directly from the Consulate/Embassy
